```Duties```
- Provide direct social work services to individuals, families, and groups in a variety of settings
- Conduct assessments to identify client needs and develop appropriate intervention plans
- Collaborate with interdisciplinary teams to coordinate and implement comprehensive care plans
- Assist clients in accessing community resources and support services
- Advocate for clients' rights and ensure their needs are met within legal and ethical guidelines
- Provide crisis intervention and support during emergency situations
- Conduct home visits to assess living conditions and provide necessary interventions
- Document all client interactions, assessments, and interventions accurately and timely
```Skills```
- Strong knowledge of Medicare regulations and guidelines
- Proficient in crisis intervention techniques and strategies
- Excellent understanding of social work principles, theories, and practices
- Experience with motivational interviewing to promote behavior change
- Knowledge of Elder welfare policies and procedures
- Familiarity with workforce development programs and resources
- Understanding of behavioral health issues and treatment modalities
- Proficient in crisis management techniques to deescalate high-stress situations
