MDS Coordinator

Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations
- Ensure accurate and timely completion of MDS assessments, including assessment scheduling, data collection, and submission
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Review medical records, interview residents, and consult with healthcare providers to gather relevant information for accurate assessment completion
- Utilize knowledge of DRG (Diagnosis Related Group) coding and Medicare guidelines to accurately assign appropriate codes and classifications
- Monitor and track changes in resident conditions to ensure timely updates to MDS assessments
- Collaborate with case management and primary care teams to develop individualized care plans based on MDS assessment findings
- Maintain confidentiality of resident information in compliance with HIPAA regulations
- Stay updated on changes in regulations and guidelines related to MDS assessments and implement necessary changes in processes

Qualifications:
- Bachelor's degree in nursing or related field preferred
- Previous experience as an MDS Coordinator or in a similar role within an acute care or long-term care setting
- Strong knowledge of MDS assessment process, DRG coding, Medicare guidelines, and case management principles
- Familiarity with medical terminology and ability to interpret medical records
- Excellent attention to detail and ability to accurately collect, analyze, and interpret data
- Strong organizational skills and ability to prioritize tasks effectively
- Proficient computer skills, including experience working with electronic medical record systems
- Ability to work collaboratively with interdisciplinary teams and communicate effectively with healthcare providers
